Physicals Must Be on the Aktivate Website: www.aktivate.com
To tryout and/or participate in any of these sports you must have a physical exam and a completed sports physical form. All forms must be completed on the Aktivate site prior to trying out for a sport. All forms will be done electronically except for the EL2 Form completed by your doctor. Once that form is completed, you will bee to upload it to your account on Aktivate.

- EL2 Form MUST BE UPLOADED to your Aktivate account with your electronic forms.
Physical exams are valid for ONE calendar year. The physical exam must be valid for the completion of the season. You may have your physical exam done by:
- Your private MD.
- A walk-in medical center.
- The Student Health Center at Glenridge Middle School. Schedule an appointment for a free physical by calling (407) 623-1415 ext 5072245
